I am so thankful for this amazing service. Caring for a client with Dementia starts to become quite a challenge, with transporting to GP appointments, especially when the client is unwell and starts to have problems with mobility. I decided to call Perth Home GP last week. The receptionist Judy was very helpful and really nice to talk to, she called me back to confirm Dr Mike was available to visit. Dr Mike has now visited twice and has followed up on our clients well being, with a tele health appointment. He was very thorough and ordered some blood test to be done with Saturn Pathology, they are also a home visiting blood collection service, in Perth. I have decided to register for future home visits with Perth Home GP. This will enable us to care for our client at home for as long as possible, with support from Dr Mike and Dr Sarah and their lovely nurses. Many Thanks, Tammy

How wonderful is it to have a concierge Doctor available in downtown Cannington??!! Dr Dave Evans is now Mum's permanent GP and his care and service is second to none. If you have a loved one that needs to have regular care and it is difficult for so many reasons to get them out of the house for an appointment, I tell you it is worth the extra $ to have a GP come to you!! And the same one each time. Can't recommend Perth Home GP enough. Our parents are worth every cent of this service!!

Our local GPs in Perth provide a personal touch with the convenience of a 24/7 door-to-door service, ensuring you receive consistent, quality care throughout the day and into the night. Booking a private consultation with us means you can enjoy the standard, comprehensive care of a general practitioner, right in your living room. We specialize in family and elderly care, mirroring the full spectrum of services you’d expect at GP clinics, but with the added benefit of a familiar face and the ease of staying at home.

At Perth Home GP, we pride ourselves on timely medical attention.  Our goal is to see our patient within 2-3 hours of booking; however this is dependent on patient demand and the patient’s geographical location.  We would aim to see to your needs faster than if you were to attend a medical centre clinic.

Please prepare:
– Current list of your medications, medical problems, and allergies.
– Your Medicare/DVA/Concession cards.
– Card for payment.

Please also turn on an outside light if your visit is taking place after the sun has gone down, and put any pets away when the doctor arrives.

Additionally, please advise us if the patient will require translation or hearing-impaired services.

For immediate care, our GPs provide a selection of starter medications to commence your treatment during the visit, followed by a script for any additional medication required.

For chronic health conditions our GPs will provide you with a script for you to take to your nearby pharmacy (similar to any medical clinic).

Certainly, our GPs can issue necessary medical and carer certificates during your visit, including certificates to facilitate your return to work.

Of course. Please advise us when booking your GP visit if the patient requires translation or hearing-impaired services.

We provide translation services through the Translating and Interpreting Service (TIS), and assistance can be provided for hearing-impaired patients through the National Relay Service (NRS).

To cancel, please call 1300 815 321 and select the option to cancel. If you have booked online you also have the option to cancel on the home page in the patient app.

If you cancel less than 2 hours before your telehealth consultation time, or the start of your provisional Home Visit estimated arrival time you will be charged a $50 cancellation fee. You will not be able to claim this back from Medicare.

If your doctor sends you a more accurate home visit estimated arrival time later than your provisional arrival time, you can cancel at no cost.

If you are not present at the time of your consultation, or you do not wish to go ahead with your home visit consultation when the doctor arrives, you will be charged a Did Not Attend (DNA) fee.

The DNA fee for telehealth is $50. The DNA fee for home visits is $150.

Important information

If you’re enrolled in Medicare you’re eligible for the Extended Medicare Safety Net (EMSN). Once your out-of-pocket medical expenses reach the threshold (below) in a calendar year, Medicare will cover up to 80% of any further out-of-pocket expenses you have for out-of-hospital medical services that are subsidised under the MBS. You don’t need to register if you are an individual with no dependants. Medicare keeps a tally of all your out of pocket expenses and gap amounts. If you are part of a family or couple you will need to register online – that way you will reach the threshold sooner. You only have to register once.

Dr Juan Stephen

Juan is a Perth local who graduated from The University of Notre Dame, Fremantle, and later achieved Fellowship with the Royal Australian College of General Practitioners. Alongside his civilian work in general practice, he serves as a Medical Officer with the Royal Australian Air Force, where he combines his interests in aviation medicine, human performance, and clinical practice.

He has a strong interest in musculoskeletal medicine, metabolic health, weight management, and preventive medicine, with a focus on helping patients achieve lasting improvements through evidence-based and sustainable approaches. Outside of medicine, Juan enjoys spending time with his wife, daughter and dog, travelling, and playing soccer.

Dr Sandeep Kulkarni

Sandeep is a Perth local who graduated from the University of Western Australia in 2018 and then later obtained Fellowship with the Australian College of General Practitioners.

He enjoys all areas of general practice, with special interests in chronic disease and metabolic medicine. Outside of medicine he enjoys travelling and exploring new parts of the world. He also quite likes a good book.

Dr Abubakar Bham

Dr Bham grew up in Gloucestershire in the UK and originally qualified as a pharmacist before studying medicine at Leicester University, graduating in 2010. He completed his GP training in Gloucestershire before moving to WA in 2017.

He has developed specialist interests in skin cancer medicine, metabolic health, men’s health and lifestyle medicine, and is passionate about helping patients take a proactive approach to their long-term wellbeing.

Outside of work, Dr Bham enjoys weightlifting, fishing and spending time with his wife and two children.

Dr Diviya Sritheran

Dr Diviya Sritheran is an experienced GP who grew up and trained in the UK. She moved to Perth in 2022 and is a Fellow of the Royal Australian College of General Practitioners.

She is an advocate for providing patient-centred holistic care and enjoys working with patients to achieve their health care goals. She has experience in all areas of general practice and has a particular interest in women’s health and children’s health.

Outside of medicine, she is a keen traveller and enjoys exploring outdoors with her family and friends.

Dr Cherry Wu

Dr Cherry Wu has been working in general practice in metropolitan Perth since 1990s. She is local graduate from UWA and has been a member of the RACGP for over 30 years as a fellow of the college. 

She has had experiences in all aspects of general practice and is still keen to continue learning. 

Outside of work she is a keen traveller and hiker who enjoys exploring the world. She also enjoys quiet hobbies such as reading, sewing, cooking and gardening.

Dr Averil Tam

Averil is a Perth local who completed her medical degree at The University of Western Australia in 2014 and achieved her Fellowship in General Practice in 2021.

Averil enjoys general practice, palliative care medical education and digital health. She is passionate about supporting patients to achieve their best possible quality of life and provides considered, evidence-based care tailored to each individual.

In her spare time, Averil plays ultimate frisbee, keeps a worm farm, and is a fan of the Peanuts comics.

Dr Sonu Thaker

Sonu completed his medical training at the University of Western Australia and holds postgraduate qualifications in palliative medicine and paediatric health. He provides thoughtful, patient-centred care across all areas of general practice and after-hours care.

Dr Sonu holds a Clinical Diploma in Palliative Medicine and has completed the Sydney Child Health Program (SCHP). He is multilingual, speaking English, Hindi and Gujarati, and is committed to supporting patients from all backgrounds.

Outside of medicine, Sonu enjoys travelling, photography and spending time with family and friends.

Dr Ravi Ganepola

Dr. Ravi graduated medicine from Riga Strading University in Latvia and moved to Australia in 2007. He then underwent General Practice training in Australia and obtained Fellowship of Royal Australian College of General Practitioners.

Dr. Ravi has obtained a diploma in palliative care and is experienced in all aspects of primary care in general practice including chronic disease management and mental health.

Dr Basma Shaikh

Dr Basma is an international Medical Graduate with more than 10 years of experience in Australian healthcare. She completed her fellowship in 2021 and enjoys all aspects of General Practice with Chronic Disease Management, Women and Children’s Health being her main interests.

Dr. Basma is a mother to 2 beautiful daughters and loves gardening and exploring nature.

Dr Sarah Cutts

Dr. Sarah has been a GP for over 30 years, graduating from Otago University New Zealand and gaining her FRACGP and Diploma of Child Health in Western Australia. She has worked in both rural and city practices, especially in women’s and children’s health.

Dedicated to providing holistic, patient-centred care, her passion is empowering people to achieve their personal health goals.

Outside of medicine, Sarah is an avid cyclist who enjoys the physical and mental benefits of exploring the outdoors on two wheels. As a strong advocate for active living, she encourages her patients to incorporate exercise into their routines to promote overall well being.

Dr David Evans

David trained at the University of Oxford, earning a degree in Physiological Sciences.  He completed clinical training in London before moving to Western Australia, where he has worked for 14 years.

With experience in Emergency Medicine, Obstetrics and General Practice, David was awarded the Edward Gawthorn Prize for the highest mark in WA for his GP fellowship exam. He has a special interest in palliative care, recently completing a Clinical Diploma in the field.

Outside of medicine, David enjoys spending time outdoors with his family and their Groodle, Rusty.

Dr Mike Patton

A passionate advocate for home-based care, Dr Mike is committed to improving access for patients who are housebound or face barriers attending traditional clinics. In 2015, he co-founded Airclinix, a mobile-first practice management system designed to modernise how GPs deliver care. In 2016, he launched Night Doctor, an after-hours home visit service.

Recognising that the greatest unmet need wasn’t after-hours care, but daytime support for vulnerable patients, Dr Mike went on to found Perth Home GP in 2020. His vision: to bring comprehensive, clinic-quality care directly into patients’ homes.

Outside of medicine, Mike is a proud partner and father of two. He enjoys fishing, rugby, cricket and spending quality time with his family over board games and retro video games.
